How You Opt In to Text Messages
American Dental Network patients opt in to receive text messages through one of the following methods:
- Paper patient intake form completed at your first visit or when updating your patient information. The intake form includes an unchecked SMS consent checkbox that you may affirmatively check to opt in.
- Verbal confirmation at the front desk, which is documented in your patient record by our team.
Consent is never a condition of receiving dental care. You may decline SMS communications at any time without any impact to your treatment, scheduling, or billing.
Consent Language Presented at Opt-In
At the point of opt-in, patients are presented with the following consent statement:
☐ I agree to receive appointment reminders, confirmations, reschedule requests, pre-visit instructions, post-visit follow-up, treatment plan updates, and billing notifications from American Dental Network at the mobile number I provided.
Message and data rates may apply. Message frequency varies based on my appointments and care needs. Reply STOP to opt out at any time, or reply HELP for help. Consent is not a condition of receiving dental care or any purchase.
